La Tarjeta de Operación deberá contener la siguiente información

a)

DATOS DE LA EMPRESA: Razón social, sede, radio de acción, y/o zona de operación.

b)

DATOS DEL VEHÍCULO: Clase, marca, modelo, número de motor, capacidad, placas.

c)

Fecha de vencimiento, numeración consecutiva, firma y sello de la oficina expedidora.
