Anexos a la solicitud de rehabilitación. A la solicitud de rehabilitación se anexarán:

1. Dos declaraciones, por lo menos, de personas reconocidamente honorables, sobre la conducta observada después de la condena.

2. Certificado de la autoridad bajo cuya vigilancia hubiere estado el peticionario en el período de prueba de la condena o libertad condicionales o libertad vigilada, si fuere el caso.

3. Comprobación del pago de los perjuicios civiles, o de los motivos que justifiquen la falta de pago
