De la licencia sanitaria de transporte. Los interesados en obtener licencia sanitaria para empresas dedicadas al transporte de plaguicidas, deberán presentar ante la División de Saneamiento Ambiental del Servicio Seccional de Salud de la sede de la empresa transportadora los siguientes documentos que acrediten el cumplimiento de los requisitos establecidos en el Capítulo VIII de la presente disposición

Solicitud escrita que contenga la información a que se refieren los literales a), b). g), i) y k) del artículo 131 y los documentos a que se refieren los literales a), b), d) y e) del artículo 132 del presente Decreto;
