La solicitud de inscripción será presentada en formulario en el cual el jefe de personal o quien haga sus veces, en la entidad u organismo en donde el empleado presta sus servicios, haga constar, bajo la gravedad del juramento, que el empleado participó en un concurso abierto, figuraba entre los tres (3) primeros puestos de la correspondiente lista de elegibles al momento del nombramiento en período de prueba y obtuvo calificación de servicios satisfactoria para dicho período.
