°. Contratos de medicina prepagada y afiliaciones a planes de salud. Son documentos equivalentes a la factura los contratos de medicina prepagada, sus planes complementarias y las afiliaciones a planes obligatorios de salud, los cuales cumplirán con los siguientes requisitos

Razón Social y NIT de la empresa de medicina prepagada o E.P.S.

2.

Numeración consecutiva.

3.

Fecha de expedición.

4.

Descripción específica o genérica del servicio.

5.

Valor de la operación.
