Acta de transporte de muestras . Cuando concluya el proceso de toma de muestras, el responsable, completará un formulario del acta de transporte de muestras, en la que se incluirá la relación de los códigos de todo el material utilizado. Asimismo, se indicarán los nombres, apellidos y firma del responsable de la toma de muestras, que será verificado en el laboratorio. En sobre dirigido al laboratorio, el responsable de la toma de muestras introducirá las copias de las actas de control al dopaje, destinadas a dicho organismo, así como el acta de transporte de muestras con la relación general de códigos.
