El diploma que acredita el Otorgamiento de la Medalla Militar "HERIDO EN ACCIÓN" debe ser elaborado en papel pergamino o cartulina blanco, de las siguientes dimensiones: Treinta y cinco (35) centímetros de largo por veinticinco (25) centímetros de ancho, con el dibujo del anverso de la medalla al lado izquierdo superior y el reverso al lado derecho superior y en el centro la siguiente leyenda: 5. Medalla servicios distinguidos en "orden público"
