SOLICITUD POR CONDUCTO DE LA CAJA DE VIVIENDA MILITAR. Cuando la solicitud del anticipo de cesantía se haga por conducto de la Caja de Vivienda Militar, el interesado deberá presentar los documentos que esta entidad exija para tramitarla a la Dirección General, acompañada de los siguientes documentos

a)

Memorial petitorio de anticipo de cesantía, con indicación de su cuantía y del fin al cual será destinado;

b)

Constancia sobre el préstamo que la Caja haya otorgado al interesado;

c)

Certificación sobre los haberes devengados por el interesado en el último mes;

d)

Certificación sobre el tiempo de servicio en la Policía Nacional;

e)

Autorización conferida por el interesado a la Caja de Vivienda Militar, para solicitar y obtener el pago del anticipo;

f)

Fotocopia autenticada de la cédula de ciudadanía.
