Contenido del formulario único o integrado. El Ministerio de Salud y Protección Social señalará el diseño y contenido del formulario único o integrado, bajo su coordinación se establecerá un Comité, integrado por las Superintendencias Financiera de Colombia, Nacional de Salud y del Subsidio Familiar, así como por las entidades acreedoras y recaudadoras de los aportes al Sistema de Seguridad Social Integral y de los aportes parafiscales, el cual realizará recomendaciones sobre el diseño y contenido del formulario.
