Es un instrumento del proceso para definir la evaluación y clasificación anual, es diligenciado por las autoridades evaluadora y revisora. Excepcionalmente puede ser modificado por las juntas clasificadoras. Concreta y define el desempeño profesional. Para su elaboración, diligenciamiento y trámite se observarán los parámetros que para el efecto disponga el Comando General de las Fuerzas Militares
