Debe ser elaborado en papel cartulina o pergamino blanco, de treinta y cinco (35) por veinticinco (25) centímetros de lado, con el Escudo Nacional a colores, en la parte superior y con la siguiente leyenda: Medalla de tiempo de servicio. El Ministro (Comandante) Por Resolución (Disposición) número........ del...... de...... de 19...... Confirió al: La Medalla........................ años de servicio....................... El diploma de la Medalla de treinta y cinco (35) años, llevar la firma del Ministro de Defensa Nacional y del Secretario General del Ministerio; el de treinta (30) años, la firma del Comandante General de las Fuerzas Militares y del Ayudante General; el de veinticinco (25), veinte (20) y quince (15) años, la firma del Comandante de la Fuerza y del respectivo Jefe de Personal.
