Los militares condecorados con la Orden Militar de San Mateo, cuando la porten en el uniforme, tienen derecho a honores por parte de las guardias de guarnición, de prevención, de buques de guerra, etc., para lo cual forman y presentan armas. En los actos y ceremonias oficiales se les asigna puesto de preferencia.
