º . Las solicitudes a que se refiere el artículo anterior deberán contener

Nombre, documento de identidad, país de origen y nacionalidad actual del peticionario;

Ciudad de su domicilio y dirección de su residencia;

Manifestación expresa de que se haga extensiva la nacionalidad a los hijos menores de edad, con indicación de su nombre, edad, y sexo, cuando sea del caso;

Razones en que se fundamenta su petición;

Manifestación de su último domicilio antes de haberse residenciado en el país.
