Solicitud de servicios electivos. Sin perjuicio del artículo 14 de la Ley 1751 de 2015, si para la realización de servicios de carácter electivo, ambulatorios u hospitalarios, las entidades responsables del pago de servicios de salud tienen establecido como requisito la autorización, esta será diligenciada por el prestador de servicios de salud con destino a la entidad responsable del pago, en el formato de solicitud y con los mecanismos definidos por el Ministerio de Salud y Protección Social.
