del artículo 13 del Decreto Ley 1281 de 2002, adicionado por el artículo 111 del Decreto-Ley 19 de 2012 y facultar al Ministerio de Salud y Protección Social para definir los requisitos y formatos que se deberán cumplir y diligenciar para el efecto.
