Artículo segundo . La inscripción se hará por triplicado en formularios especiales, reglamentados por el Departamento Administrativo Nacional de Estadística. Por conducto de las Secretarias Departamentales de Educación, se suministrarán dichos formularios a los Directores de los Establecimientos Educativos, que deberán consignar los siguientes datos

Nombre y dirección del plantel;

Enseñanzas que se dictan, de acuerdo con la clasificación y definición consignadas en el artículo 3º y 4º de este Decreto;

Nombre del propietario del plantel;

Nombre del Director responsable.
