Para tramitar la autorización a que se refiere el artículo anterior, el interesado en organizar la excursión de caza, deberá presentar solicitud por escrito en papel sellado anexando los siguientes datos y documentos. 1) Nombre, identificación y domicilio. 2) Si se trata de persona jurídica, razón social, prueba de su constitución y existencia y nombre, identificación y domicilio de su representante legal. 3) Especie o especies que pretende hacer objeto de caza. 4) Lugar donde se pretende desarrollar la caza. 5) Mes del año, previsto para realizar la excursión. 6) Declaración de efecto ambiental.
