°. La solicitud de inscripción se hará mediante el diligenciamiento del formulario que preparará y distribuirá la entidad contratante. En dicho formulario deberán constar de manera clara y precisa los datos o informaciones de carácter general o especial que se soliciten, así como las distintas clases o grupos que integran el registro.
