Las Personas O Entidades Obligadas A Efectuar Retención, Deberán Expedir A Solicitud Del Beneficiario, Un Certificado Que Contendrá Los Siguientes Datos: 1

º. Las personas o entidades obligadas a efectuar retención, deberán expedir a solicitud del beneficiario, un certificado que contendrá los siguientes datos

Fecha del pago;

Nombre y NIT del beneficiario;

Monto del premio;

Valor de la retención.
