Las entidades avaladas deberán certificar a las personas que cursen y aprueben el programa académico ofrecido, el cual deberá contener por lo menos la siguiente información:

Nombre de la entidad avalada.

Número y fecha de la resoluci6n de otorgamiento del aval para impartir la formación.

Nombre y documento de entidad del estudiante.

Certificación de la aprobación del programa académico respectivo.

Intensidad horaria del programa.
