º. Contenido del carné de afiliación. El carné de afiliación al Régimen del Subsidio Familiar deberá contener la siguiente información

1.

Nombre y domicilio de la respectiva Caja.

2.

Número de orden y vigencia.

3.

Nombre e identificación del afiliado.

4.

Clase de afiliado.

5.

Nombre del empleador y número de identificación tributaria (NIT).

6.

Cónyuge o compañero permanente y personas a cargo.
