Formularios de evaluación para personal civil EP-2 . Los diferentes aspectos del formulario de evaluación se agrupan en cinco (5) secciones, así: --Sección primera: Datos de identificación personal e información general. --Sección segunda: Evaluación de desempeño. --Sección tercera: Concepto autoridad evaluadora. --Sección cuarta: Concepto de autoridad revisora. --Sección quinta: Clasificación. Los formularios serán timbrados en papel bond, blanco, tamaño oficio 33 x 23 cms. el rayado será el estipulado en los Anexas 3 y 4 formas EP-1 y EP-2 respectivamente.
