En caso de pérdida o deterioro del carnet que acredite la calidad de radioaficionado, el Ministerio de Comunicaciones podrá expedir duplicado del mismo. Para el efecto se requiere

Solicitud escrita del interesado.

Adjuntar el carnet deteriorado o la denuncia de su pérdida, según el caso.

Recibo de pago a favor del Fondo de Comunicaciones por un valor equivalente a cinco (5) salarios mínimos legales diarios.

Estar a paz y salvo por todo concepto con el Ministerio de Comunicaciones.
