° Con la consignación debe acompañarse un informe que contenga los siguientes datos: 1° Nombre de la empresa; 2° Nombre y dirección de los suscriptores de teléfonos, con indicación del número de aparatos y derivaciones que hayan tenido en uso durante el mes anterior, y del número de orden que les corresponda en el directorio.
