Concepto odontológico . En lo dispuesto por la Ley 35 de 1989, artículo 1°, literal h), y demás constancias solicitadas, los conceptos emitidos por el odontólogo deberán ser por escrito y contener por lo menos los siguientes datos

1.

Lugar y fecha de expedición.

2.

Persona o entidad a la cual se dirige el certificado.

3.

Objeto o fines del certificado.

4.

Nombre e identificación del paciente.

5.

Concepto.

6.

Nombre del odontólogo.

7.

Número de la tarjeta profesional o carnet.

8.

Firma del odontólogo. (Artículo 19 del Decreto 491 de 1990)
