El titular de esta tarjeta está obligado a formular la denuncia correspondiente en caso de extravío, copia de la cual deberá remitir al Ministerio de Salud. Si esta Tarjeta es encontrada, favor enviarla a la Autoridad Sanitaria más cercana. La tarjeta profesional de médico tendrá las siguientes dimensiones: largo 9 cm., ancho 6 cm.; en fondo blanco y letras verdes.
