Inmediatamente después de llenar el formulario número 2, tal como se indica en el anterior, el Registrador Municipal deberá relacionar en el formulario número 4 el número de la cédula, los apellidos y nombres completos del ciudadano y el número de la lista. Además, llenará una tarjeta de catorce y medio centímetros por diez centímetros, destinada al archivo alfabético de la Registraduría Municipal, según al modelo que suministrará la División de Identificación.
