La adjudicación de la Medalla Cívica "Camilo Torres" será certificada por medio de un diploma que contenga la siguiente literalidad

República de Colombia Ministerio de Educación Nacional;

Medalla Cívica "Camilo Torres";

El Excelentísimo señor Presidente de la República mediante Decreto número __del __ de __ de 199( ) confirió a__ la Medalla Cívica "Camilo Torres", en categoría___; e) Lugar y fecha de entrega; g) Firma del Presidente de la República y del Ministro de Educación Nacional.

Parágrafo

En la parte superior del diploma se insertará el escudo de Colombia y el anverso y reverso de la medalla; un número de orden con la referencia del folio del libro donde quedan registrados y la refrendación del Secretario General del Ministerio de Educación.
