La demanda de adopción deberá contener:

La designación del juez a quien se dirija; 2º. El nombre, edad, domicilio o residencia del demandante; 3º. El nombre, edad, domicilio o residencia del menor que pretenda adoptarse, así como el nombre y domicilio de los padres o del guardador, salvo que se trate de menores abandonados; 4º. Los hechos y motivaciones que sirvan de fundamento a las peticiones del demandante; 5º. Los fundamentos de derecho que se invoquen; 6º. La petición de las pruebas que se pretenda hacer valer.
