Las apuestas, para ser consideradas válidas, deberán ser realizadas en el formulario oficial del juego, el cual será emitido exclusivamente por las entidades concesionistas, Este formulario será vendido por dichas entidades al concesionario debidamente autorizado y será de obligatoria utilización para el juego.
