El acta de defunción expresará: 1°. El nombre y apellido del muerto. 2°. El lugar, día y hora en que hubiere acaecido la muerte. 3°. La edad, sexo y estado civil, expresando el nombre del cónyuge, si el difunto hubiere sido casado o viudo. 4°. La nacionalidad del difunto. 5°. La última ocupación del difunto. 6°. El último domicilio. 7°. El nombre y apellido del padre y de la madre del muerto, si fueren conocidos. 8°. La causa principal de la muerte; y 9°. El nombre del médico que certificó la muerte, en su caso.
