El Instituto Nacional de Vivienda de Interés Social y Reforma Urbana -Inurbe-, diseñará y distribuirá gratuitamente un formulario único de inscripción que será utilizado por todas las entidades otorgantes del subsidio familiar de vivienda. El postulante al suscribir el formulario único de inscripción declarará bajo juramento que la información en él contenida es cierta. La entidad otorgante se reserva el derecho de verificar esta información y de aplicar en su caso las sanciones establecidas en la Ley 3º de 1991.
