Artículo quinto. Para obtener la matrícula profesional de Químico, el interesado deberán presentar ante el Consejo Profesional de Química la siguiente documentación

Solicitud suscrita por el interesado.

Fotocopia autenticada del título de Químico y certificado de registro del mismo, expedido por la autoridad competente.

Dos fotografías tamaño cédula.

Recibo de pago de los derechos de matrícula a que se refiere el literal c) del artículo 9º de la Ley 53 de 1975

Además de los anteriores, la visa de residente, cuando se trate de extranjeros.
