Formato Único de Declaración . La Unidad Administrativa Especial de Atención y Reparación a las Víctimas definirá los medios, instrumentos y mecanismos mediante los cuales se tomará la declaración, en el cual se consignarán los datos básicos que permitan la obtención, desde un enfoque diferencial, de la información necesaria para una correcta valoración y faciliten la determinación de las medidas de asistencia, atención y reparación que se adecuen al daño sufrido y las necesidades de cada víctima.
