Parágrafo

del artículo 12 del Decreto 2340 del 19 de agosto de 2003 quedará así: "

Parágrafo

El interesado deberá acreditar los requisitos señalados en los numerales 1, 2 y de este artículo, mediante el diligenciamiento del formulario de que trata el artículo 15 del presente decreto. La información consignada en el formulario por parte del solicitante del subsidio se entenderá presentada bajo la gravedad del juramento, el cual se considera prestado con la suscripción del formulario".
