Contenido del carné de afiliación. El carné de afiliación al Régimen del Subsidio Familiar deberá contener la siguiente información:

Nombre y domicilio de la respectiva Caja.

Número de orden y vigencia.

Nombre e identificación del afiliado.

Clase de afiliado.

Nombre del empleador y número de identificación tributaria (NIT).

Cónyuge o compañero permanente y personas a cargo.
