De todo pasaporte quedará en la Oficina Expedidora un formulario debidamente diligenciado, que contendrá además de las menciones que figuran en el pasaporte, las siguientes: dirección, teléfono, ocupación, nombre y nacionalidad de los padres, libreta militar y datos completos de la fisonomía del solicitante.
