La Medalla se acredita con un diploma elaborado en papel pergamino o cartulina blanca de treinta y cinco (35) centímetros de largo por veinticinco (25) centímetros de ancho, con el dibujo del anverso de la medalla al lado izquierdo superior y el del reverso al lado derecho superior y la siguiente leyenda:
