Declaración especial. En el momento de la afiliación a la Entidad Promotora de Salud seleccionada por la madre comunitaria ésta deberá certificar que no se encuentra afiliada en la actualidad al régimen contributivo en calidad de cotizante o beneficiaria, que hace parte del Programa de Hogares Comunitarios del Instituto Colombiano de Bienestar Familiar y anexará un documento de la Asociación de Padres en donde conste cuál es el valor de la bonificación mensual que recibe por concepto de los servicios prestados a la comunidad.
