Para determinar el origen de la invalidez, de la enfermedad o de la muerte, la historia clínica, o su resumen, deberá contener, además de los documentos señalados en el artículo anterior, el informe de la muerte elaborado por el empleador o por la entidad de seguridad social correspondiente. Adicionalmente y cuando la muerte fue producto de un accidente de trabajo, el acta de levantamiento de cadáver.
