La solicitud se presentará en el formulario destinado al efecto, el cual contendrá como datosmínimos, la identificación del asegurado y de las instituciones de los Países Miembros en los cuales este hubiere acreditado períodos asimilados, con indicación de las fechas entre las cuales se hubieren producido. Al formulario deberá acompañares los documentos justificativos del derechoque para cada una de las prestaciones se determinen en el mismo.
