La solicitud de inscripción debe hacerse por escrito acompañada de la correspondiente Tarjeta de Registro en donde la compañía certificará sobre la nacionalidad del Agente, número y lugar de expedición de la cédula de ciudadanía; territorio en donde ejercerá sus funciones; la constancia de que el Agente es mayor de edad y ha recibido la instrucción necesaria en el ramo o ramos a que se refiere su nombramiento; el retrato, la firma autógrafa del interesado y su dirección permanente.
