La Medalla "Al Mérito Postal" se acompañará de un diploma de treinta (30) centímetros de largo por veinticinco (25) de ancho, en cuya parte superior ostentará el escudo de armas de la República, enmarcado entre las palabras "República de Colombia" - "Ministerio de Comunicaciones", y con la siguiente leyenda: "En nombre del Gobierno de la República de Colombia, concédese la Medalla al Mérito postal, en……categoría, al señor ... ………Bogotá, ... ……de ... …de 195.."
