Decreto 2309 de 2002 · por el cual se define el Sistema Obligatorio de Garantía de Calidad de la Atención de Salud del Sistema General de Seguridad Social en Salud.

Reporte de novedades . Con el propósito de mantener actualizado el Registro Especial de Prestadores de Servicios de Salud, el Ministerio de Salud establecerá el Formulario de Reporte de Novedades, a través del cual se efectuará la actualización del dicho registro por parte de la entidad departamental o distrital de salud en su respectiva jurisdicción. Se consideran novedades en el Registro Especial de Prestadores de Servicios de Salud las siguientes

Cierre o apertura de servicios.

Cambio de representante legal o de domicilio.

Disolución o liquidación de la entidad. Este formulario no reemplaza al Formulario de Inscripción contemplado en el artículo 14 del presente decreto.
