º. La Junta de Títulos Farmacéuticos entregará a quienes reúnan los anteriores requisitos el siguiente permiso impreso en papel sellado. El señor… con cédula de ciudadanía número… expedida… De… años de edad, puede dirigir la farmacia… Ubicada en el Municipio… Corregimiento… Caserío… y en tinta roja la siguiente leyenda: Este permiso caduca cuando en el Municipio… Corregimiento... Caserío… se instalen más de dos farmacias dirigidas por farmacéuticos y será retirado y enviado a la Junta de Títulos Farmacéuticos, por las respectivas autoridades. Además llevará el retrato tamaño pasaporte del interesado.
