El acta de defunción expresara:

1º El nombre y el apellido del muerto;

2º El día y la hora en que hubiere acaecido la muerte;

3º La edad, sexo y estado civil, expresando el nombre del cónyuge, si el difunto hubiere sido casado o viudo;

4º La nacionalidad

5º La ultima ocupación del difunto;

6º El ultimo domicilio;

7º El nombre y apellido del padre y la madre del muerto si fueren conocidos;

8º La causa principal de la muerte, y

9º El nombre del médico que certificó la muerte en su caso.
