Formulario EP-1 . Sirve tanto para oficiales como suboficiales. Al elaborar la evaluación se debe marcar con una equis (x), según a quien corresponda el rectángulo que aparece en el encabezamiento del formulario.

Parágrafo

Cuando se trate de oficiales o suboficiales que se desempeñen como alumnos de cursos para ascenso o especialización, se evaluará el área I y el literal b) del área III, caso en el cual se adjunta debidamente elaborado el formulario RA (Ver Anexo número 5)
