Diploma. Cada Medalla va acompañada de un diploma credencial firmado por el Comandante de Brigada, Fuerza Naval, Comando Aéreo, Comandante del Cuerpo o Tropa, buque o establecimiento terrestre al cual pertenezca el agraciado. Se elabora en papel cartulina o pergamino blanco de treinta y cinco (35) por veinticinco (25) centímetros de ancho, con el Escudo Nacional en colores, en la parte superior y con la siguiente leyenda CAPÍTULO V Condecoraciones por Servicios Distinguidos prestados a la Institución Militar
