La solicitud de libertad condicional debe ir acompañada de la resolución favorable del consejo de disciplina, o en su defecto de la proferida por el director del establecimiento carcelario, de la copia de la cartilla biográfica y de los demás documentos que prueben los requisitos exigidos por este código.
