ARTICULO 39. Estas solicitudes se harán en igual forma que las solicitudes de revalidación, pero anexando los siguientes documentos

1.

POR PERDIDA DE LA LICENCIA

a)

Copia de la denuncia presentada ante la autoridad competente.

b)

Recibo de pago de los derechos de licencia.

c)

Recibo de pago por valor del duplicado.

2.

POR DETERIORO DE LA LICENCIA

a)

Original de la licencia expedida.

b)

Recibo de pago de los derechos de la licencia.

c)

Recibo de pago por valor del duplicado. TITULO QUINTO. DE LAS FUNCIONES Y OBLIGACIONES DEL PERSONAL A BORDO. CAPITULO UNICO
